There could be additional fees that would be required depending on the city and state in which the person passes away. And also some other information like the vital statistics in order to help complete all the necessary documents for cremation.

Yes, each state has their individual requirements. In Minnesota there are 3 forms required for the cremation process. First is the signed county cremation permit which is a document signed by both the physician and county medical examiner. Next is the Authorization for Cremation which is signed by the legal next of kin. A Self Authorization, a form that can be pre signed by the deceased authorizing cremation, can replace the Authorization for Cremation form. Finally, a Disposition Permit, a document issued by the funeral home when the death certificate is initiated, is required.
Our company also requires a fourth document, the Confirmation of Identification. This form states that the family has viewed or declines to view the deceased. If they decline a photo, then another form of identification must be provided. The funeral director uses this form of identification to confirm correct identification prior to cremation.
